New Horizons Village is a division of New Horizons, Inc. which is a 501(c)3 nonprofit organization. For the past 36 years, New Horizons Village has been providing affordable housing and support services to adults who have physical disabilities.  Our 26-acre campus is located in a quiet residential neighborhood in the Unionville section of Farmington.  

New Horizons Village offers a variety of support services to the 101 residents including:

• Care Services: Residents at New Horizons Village hire their own personal Care Attendants (PCAs) in order to assist them with their specific needs. However, the residents have peace of mind knowing that New Horizons Village offers a backup care service in situations when their PCAs are not available. Residents can call our office 24 hours a day/7 days a week if they have unanticipated needs. Many of our tenants are unable to get out of bed or tend to their basic needs without assistance so back up care truly allows them to live independently rather than in an institutional setting. There is also an emergency call for aid system in every apartment in case a resident needs immediate help.

• Transportation: New Horizons Village offers a transportation services to all of the 101 individuals that live at New Horizons Village. Most of the residents are not able to drive because of their disability; therefore, this service is critical. On a monthly basis, New Horizons Village's transportation program makes numerous trips to fulfill residents' health needs. There are trips to pharmacies to obtain medications that enable a higher quality of life, to physical therapists who help improve movement and manage pain, to food banks that provide local produce and other healthy food options, as well as to many other health related organizations. New Horizons Village is not located on a public bus route; therefore, our transportation program is a critical service.

• Health Service Coordinators: New Horizons Village has a nurse on staff. This individual educates residents on preventive health care. She also provides information and referrals to community medical resources. In addition, the nurse acts as the communication liaison with the area health service providers at the tenant’s request.

The residents rely on these services and utilize them on a daily basis. New Horizons Village provides a very supportive environment.
